A guide to using the Assessments section in Access Provider Gateway to complete, track, and review provider evaluations for quality, compliance, and performance.

The Assessments section in Access Provider Gateway lets you conduct, record, and review quality and compliance assessments for your care providers. You can use digital tools to standardise scoring, attach evidence, and track performance over time, supporting risk management, improvement planning, and regulatory requirements.

⚠️ Important: You'll need Manager or Administrator permissions to create and submit assessments.

How to complete an assessment

Open the Assessments section

  1. Click Provider Gateway, then click Assessments.

  2. Click the provider you want to assess.

Choose or create a template

  1. Click Select template to choose from your available templates.

  2. If you need a custom template, click Create template and follow the on-screen steps.

📌 Note: Templates can be tailored to match quality, safety, or compliance criteria depending on your organisation's requirements.

Complete the assessment

  1. Fill in the required fields, scores, and comments.

  2. Click Add evidence to upload any supporting files or images.

  3. Click Save progress at any point to avoid losing your work.

🤓 Tip: Complete all mandatory fields before submitting to avoid validation errors.

Submit the assessment

  1. Review your responses to ensure all mandatory sections are complete.

  2. Click Submit to save the assessment and register completion.

⚠️ Important: Once submitted, you cannot edit the assessment. Make sure all information is accurate before clicking Submit.

Review history and results

  1. Click History to view completed assessments for the provider.

  2. Click an assessment to open and review it.

  3. Click Analytics to view summarised results and performance trends.

Set follow-up actions

  1. Click Add follow-up if issues are identified during the assessment.

  2. Enter your notes, reminders, or corrective actions.

  3. Click Save to confirm.

Good practice

  • Use standardised templates to ensure consistency across all provider assessments.

  • Attach evidence such as images or documents to support your findings.

  • Review assessment history before starting a new assessment to identify recurring issues.

  • Set follow-up actions immediately after submitting to ensure issues are tracked and resolved.

Poor practice

  • Completing assessments without reviewing previous history, which can lead to missed patterns or recurring issues.

  • Submitting assessments without attaching supporting evidence, which weakens the audit trail.

  • Ignoring automated alerts for overdue assessments, which can result in compliance gaps.
