Create an on-site assessment

Follow the steps to create an on-site assessment, enabling information to be captured and recorded during in-person visits.

You can create an on-site assessment using the five-step wizard in Provider Manager. Before you start, make sure you have a published question template ready — only published templates can be attached to an assessment.

⚠️ Important: You must have a published question template before you can create an assessment. See the publish a question template article if you haven't done this yet.


Find the On-Site Assessments area

To get started:

  1. Click Quality Assurance in the left-hand navigation, then click Manage Assessments.

  2. Click On-Site Assessments.

You'll see your assessments organised by status. Use the status filters along the top to focus on the assessments you need:

Status (H3)

Ready to Assess

Created and waiting to be started.

In Progress

Being worked through by an assessor.

With Provider

Submitted and sitting with the provider to respond.

With LA

Returned to your team to review and respond.

Agreed

Accepted by the provider; locked from further changes.

Complete

Finalised, with the final rating shared.

Archived

Tidied away from your day-to-day list.

🤓 Tip: Switch between card view and list view using the icons on the right — the list view adds handy sort options. Use the Filter and search tools to find any assessment quickly when you have a lot on the go.


Create the assessment

To create a new assessment, click Create Assessment. A five-step wizard guides you through the rest.

Step 1: select a provider

  1. Choose the provider you're assessing from the list.

Step 2: choose the assessment type

  1. Select On-Site as the assessment type.

📌 Note: This guide covers on-site assessments only. A separate guide covers self-assessments.

Step 3: select your template

  1. Select the published question template you want to use.

Step 4: add assessment details

  1. Give the assessment a name — this defaults to the provider's name plus the template name, but you can change it.

  2. Add any notes relevant to the assessment.

📌 Note: You'll see a quick summary of the provider and how many domains, standards and questions are included.

Step 5: preview and create

  1. Review the preview to check everything looks right.

  2. Click Create Assessment.

Your new assessment now appears with a Ready to Assess status and a clear On-Site badge. The card shows progress, rating, when it was started and last updated, and who created it.

🤓 Tip: You can delete the assessment from the card if you need to start again.
