The Form Builder is a visual canvas that lets you drag and drop fields to design your form. You can choose from a range of field types, configure how each one looks and behaves, and preview your form at any point before saving.
Add fields to your form
To build your form, drag fields from the Available Fields panel onto the canvas.
Open Form Builder from the Campaigns menu in the left-hand navigation.
Find the field you want in the Available Fields panel on the left.
Drag the field onto the form canvas, or click it to add it to the bottom of your form.
Repeat until you have added all the fields you need.
🤓 Tip: You can add as many fields as you like, in any order.
Configure each field
Once you have added a field, you can configure how it looks and behaves using the Properties panel.
Click any field on the canvas to open its settings in the Properties panel on the right.
Update the settings you need:
Field label: the question or prompt the provider sees.
Placeholder text: faint example text shown inside the field.
Help text: short guidance shown beneath the field.
Required: turn on to make the field mandatory.
Repeat for each field on your form.
Name your form
Give your form a name so you can identify it easily in the Form Builder.
Click Form Settings.
Enter a name for your form, for example, 'Provider Feedback Form'.
Click Save.
📌 Note: The name you enter appears at the top of the form when your providers open it.
Preview your form
You can preview your form at any point to see exactly how it will look to your providers.
Click Preview Form.
Select a screen size to preview across different devices:
Desktop.
Mobile.
Tablet portrait.
Tablet landscape.

Basic fields

Short Text | Short answers such as names or reference numbers. |
Long Text | Longer answers such as descriptions or comments. |
Number | Numerical values such as amounts or counts. |
Date | Date entries. |

Selection fields

Dropdown | Lets providers choose one option from a list. |
Radio Buttons | Lets providers choose one option from a visible set. |
Checkboxes | Lets providers select one or more options. |
Yes/No Toggle | A simple yes or no answer. |

Advanced fields

File Upload | Asks providers to attach a document. |
Signature Capture | Lets a provider sign directly on the form. |
Rating Scale | Collects feedback or net promoter scores. |
Calculated Field | Automatically works out a value from other number fields. |
Layout elements

Brand Image | Adds your Local Authority logo or other imagery. |
Instructional Text | Adds helpful guidance for the provider. |
Divider | Breaks long forms into clear, manageable sections. |
Page Break | Splits the form across multiple pages. |
