The AI Form Builder is the fastest way to create a form in Access Provider Manager. Instead of adding fields manually, you can describe the form you want in plain English, or upload an existing document, and the AI builds it for you. A form that previously took around eight days to build through a support request can now be created in about a minute, completely self-serve.
⚠️ Important: You'll need to have access to the Form Builder in Access Provider Manager before following these steps
Option A: Build a form from a text prompt
To build a form using a text prompt, follow these steps.
In Access Provider Manager, click Campaigns in the left-hand navigation, then click Form Builder.
Open the AI Form Builder panel, then select Text Prompt.
In the text box, describe the form you want — for example, "Build a simple survey form for my providers."
Click Generate and wait for the AI to build your form.
Click Preview to review the generated form.
If you need to make changes, enter a follow-up instruction in the text box — for example, ask the AI to include conditional logic or calculated fields — then click Generate again.
📌 Note: The more detail you include in your prompt, the more accurate the generated form will be. You can include references to specific field types, such as dropdowns or calculated fields.
Option B: Build a form from an uploaded document
If you already have your form structure in an existing file, you can upload it and the AI will build the form for you — no rekeying required.
In Access Provider Manager, click Campaigns in the left-hand navigation, then click Form Builder.
Open the AI Form Builder panel, then select the option to upload a document.
Select your file — for example, an existing spreadsheet of your quarterly standards campaign. Supported formats include spreadsheets, Portable Document Format (PDF) files, and Comma-Separated Values (CSV) files.
Click Generate from document and wait for the AI to analyse the file and build the matching fields onto your canvas.
Click Preview to review the generated form.
Make any adjustments needed before saving and publishing.
Option B: Build a form from an uploaded document
If you already have your form structure in an existing file, you can upload it and the AI will build the form for you — no rekeying required.
