Save and publish a form

Steps to save and publish your completed form in Access Provider Manager so it is ready to attach to a campaign.

Once you have finished building your form in the Form Builder, you need to save it and then publish it before it can be used in a campaign. Forms start as drafts, giving you time to review and refine before they go live.


Save your form

To save your form, follow these steps.

  1. In the Form Builder, click Save Form.

  2. Open Form Settings and give your form a name — for example, 'Provider Feedback Form'. The name appears at the top of the form when your providers open it.

  3. Click Save.

📌 Note: Your saved forms appear under the Templates tab in the Form Builder. A newly saved form starts as a draft.

Publish your form

To publish your form and make it available to attach to a campaign, follow these steps.

  1. In the Form Builder, click the Templates tab.

  2. Open the form you want to publish.

  3. Click Publish.

⚠️ Important: Only published forms can be attached to a campaign. Keep a form as a draft while you are still working on it, then publish it when it is ready to send.
