Private assessor notes let you attach an internal note to any question on an assessment. It sits alongside Help Text, Rating and Evidence on the question.
⚠️ Important: private assessor notes are visible only inside Provider Manager. The provider never sees them, on Provider Gateway or anywhere else.
What it's for

How to add a note
Open the question
Open the assessment and go to the question you want to note.
Add the note
Expand Private assessor notes.
Type your note.
🤓 Tip: the note saves along with the rest of the assessment. There's no separate save step.
Good and poor practice
Good practice
Use it to record the reasoning behind a rating, especially anything borderline.
Use it for reminders that only matter internally, like a follow-up check on the next visit.
Poor practice
Relying on it to communicate anything to the provider. It is never shown to them, so use Evidence or the shared record for that.
Treating it as a replacement for Evidence when a rating needs supporting documentation.
Summary
Private assessor notes give you a place to record internal-only context on any assessment question, without it ever reaching the provider. It's a new field, not something migrated from PAMMS, so start adding notes fresh in Provider Manager.
